(Gd) Gadolinium is a Lanthanide element that is paramagnetic in its trivalent state.
This paramagnetic substance is used for MR imaging because of the effect of strongly decreasing the T1 relaxation times of the tissues to which gadolinium has access. When injected during magnetic resonance imaging, gadolinium will tend to change signal intensities by shortening the T1 time in its surroundings.
The relaxivity of gadolinium is an important measure of its efficacy, which is dependent on the chemical properties of the complex. The gadolinium ion cannot be used in its chloride, sulfate, or acetate forms because of poor tolerance and low solubility in water in the neutral pH range. Although toxic by itself, gadolinium can be given safely in a chelated form such as DTPA, that still retains much of its strong effect on relaxation times ( relaxivity).

Zeolites are hydrated aluminosilicates with large holes that can provide a temporary home for guest molecules like gadolinium. An agent currently in Phase III clinical trials that selectively targets diseased cells because they have increased rates of metabolism. Once inside the diseased cell, motexafin gadolinium may work to disrupt its energy production and the ability to repair itself. This biolocalization can be confirmed because the compound can be seen via MRI. Motexafin gadolinium accumulates in diseased cells with repeat doses and remains in the cells for days.

(NSF) Nephrogenic systemic fibrosis is a rare and highly debilitating disorder that involves extensive thickening and hardening of the skin with fibrotic nodules and plaques.
MRI contrast media have very low side effects, but accumulating data indicate that gadolinium-based contrast agents increase the risk for the development of NSF among patients with severe renal insufficiency or renal dysfunction due to the hepato-renal syndrome or in the perioperative liver transplantation period.
Due to this reason, gadolinium contrast agents are now considered contraindicated in patients with an estimated glomerular filtration rate fewer than 30 mL/min/1.73m 2.
In these patients, avoid use of gadolinium-based contrast agents unless the diagnostic information is essential and not available with non-contrast enhanced magnetic resonance imaging ( MRI).
Recognized or possibly associated factors for NSF:
•
high dose of erythropoietin;
•
high serum phosphate levels;
•
high serum calcium levels;
•
major surgery, infection, vascular event;
•
history of hypothyroidism;
When administering a gadolinium-based contrast agent, do not exceed the recommended dose and allow a sufficient period of time for elimination of the contrast medium from the body prior to any readminstration. Screen all patients for renal dysfunction by obtaining a history and/or laboratory tests.
