Spectroscopy
 
Edward Purcell and Felix Bloch discovered the basic of spectroscopy in 1946 (see MRI History). Nuclear magnetic resonance spectroscopy (NMR Spectroscopy or MRS) is an analytical tool, based on nuclei that have a spin (nuclei with an odd number of neutrons and/or protons) like 1H, 13C, 17O, 19F, 31P etc.
Through nuclear magnetic principles as precession, chemical shift, spin spin coupling etc., the analysis of the content, purity, and molecular structure of a sample is possible. The spectrum produced by this process contains a number of peaks; the highs and the positions of these peaks allow the exact analysis. Unknown compounds can be matched against spectral libraries. Even very complex organic compounds as enzymes and proteins can be determined. For the wide uses of NMR spectroscopy (from mineralogy to medicine) there is a variety of different techniques available.

Nephrogenic Systemic FibrosisForum -

(NSF) Nephrogenic systemic fibrosis is a rare and highly debilitating disorder that involves extensive thickening and hardening of the skin with fibrotic nodules and plaques.
MRI contrast media have very low side effects, but accumulating data indicate that gadolinium-based contrast agents increase the risk for the development of NSF among patients with severe renal insufficiency or renal dysfunction due to the hepato-renal syndrome or in the perioperative liver transplantation period.
Due to this reason, gadolinium contrast agents are now considered contraindicated in patients with an estimated glomerular filtration rate fewer than 30 mL/min/1.73m2. In these patients, avoid use of gadolinium-based contrast agents unless the diagnostic information is essential and not available with non-contrast enhanced magnetic resonance imaging (MRI).

Recognized or possibly associated factors for NSF:
Severe renal failure;
high dose of gadolinium chelate;
high dose of erythropoietin;
high serum phosphate levels;
high serum calcium levels;
major surgery, infection, vascular event;
history of hypothyroidism;
metabolic acidosis.

When administering a gadolinium-based contrast agent, do not exceed the recommended dose and allow a sufficient period of time for elimination of the contrast medium from the body prior to any readminstration. Screen all patients for renal dysfunction by obtaining a history and/or laboratory tests.

(MR mammography) Magnetic resonance imaging of the breast is particularly useful in evaluation of newly diagnosed breast cancer, in women whose breast tissue is mammographically very dense and for screening in women with a high lifetime risk of breast cancer because of their family history or genetic disposition.
Breast MRI can be performed on all standard whole body magnets at a field strength of 0.5 T - 1.5 Tesla. Powerful gradient strengths over 15 mT/m will help to improve the balance between spatial resolution, scanning speed, and volume coverage. The use of a dedicated bilateral breast coil is obligatory.
Malignant lesions release angiogenic factors that increase local vessel density and vessel permeability. Breast cancer is detectable due to the strong enhancement in dynamic breast imaging that peaks early (about 1-2 min.) after contrast medium injection. If breast cancer is suspected, a breast biopsy may be necessary to secure the diagnosis.

Requirements in breast MRI procedures:
•
Both breasts must be measured without gaps.
•
Temporal resolution should be sufficient to allow early imaging after contrast agent with dynamic imaging every 60-120 sec.
•
For the best possible detection of enhancement fat signal should be eliminated either by image subtraction or by spectrally selective fat saturation.
•
Thin slices are necessary to assure absence of partial volume effects.
•
Imaging should be performed with a spatial resolution in plane less than 1 mm.
